LIFE PLANNING SERVICES

Life planning services acts as a link to a variety of adult services available that can help the process of transitioning young people with developmental disabilities to adult living.  By providing planning and referral services to families who have a child with a developmental disability, life planning services identifies an individual’s capacities, interests and dreams, and develops strategies to realize them. For young people, this plan becomes a guide as they prepare for life after high school.

Life planning services connects families to supports such as

•    Vocational and Career Training
•    Service Coordination
•    Parent Empowerment Training and Family Support Groups
•    Financial Planning: Understanding Government
•    Entitlements, Benefits, and Trusts
•    Health and Medical Assistance
•    Recreational Opportunities
•    Respite Services
•    Residential Opportunities
•    Community Supports
•    Guardianship
